Job summary

Alzheimer’s Community Care is seeking a Wandering Prevention Coordinator to safeguard patients with Alzheimer’s and related dementias. You will maintain patient/caregiver files, enter data, and collaborate with Law Enforcement and SafetyNet Tracking Systems to ensure safe service delivery.

Responsibilities include coordinating referrals, scheduling enrollment visits, managing communications, and tracking equipment and supplies.

Qualifications

• High school diploma or equivalent with administrative experience.
• Three years of professional administrative experience, preferably in health care.
• Valid driver license and reliable transportation.

Responsibilities

• Coordinate referrals and enrollments with caregivers or designees.
• Schedule enrollment appointments at centers, office, or patient homes.
• Manage inquiries and follow up with caregivers regarding services.
• Schedule monthly maintenance visits for equipment in patient environments.
• Document monthly worksheets for reporting needs.
• Collaborate with SDS to ensure service delivery and inventory management.
• Monitor supplies and track enrollment materials and equipment accessories.
• Enter patient and caregiver data into EMR/ADS and SafetyNet systems.
• Process documents and communicate enrollment updates to team and external entities.
• Maintain caregiver and SDS tracking logs and resolve missing information.
• Oversee equipment replacement and assist with troubleshooting.
