Job summary

MassAbility seeks a Waiver Program Coordinator to develop individualized service plans (ISPs) for ABI-N and MFP-CL enrollees statewide, coordinating an interdisciplinary team to tailor services and supports in a person-centered plan. The role requires travel across Massachusetts and close collaboration with guardians, providers, and MassHealth resources.

The coordinator will monitor service delivery, assist with referrals, and ensure timely completion of home modifications.

Qualifications

• Ability to travel statewide.
• Excellent communication skills, both oral and written.
• Ability to self-organize and manage priorities.
• Comfort with remote technology (computer, VPN, remote calling).
• Familiarity with needs of individuals with disabilities living in the community.

Responsibilities

• Engage enrollees/participants and guardians in person-centered planning.
• Meet in-person for specialized assessments pre-discharge with professionals.
• Develop a person-centered plan (ISP) including services and community resources.
• Coordinate with families and MassAbility staff to ensure home modifications meet needs.
• Coordinate interdisciplinary team process for each Person-Centered Plan.
• Update assessments and arrange for special assessments as needed.
• Safeguard waiver services and ensure delivery and satisfaction.
• Arrange referrals to MassHealth LTSS and other services; monitor service delivery.
• Conduct outreach to nursing facilities and rehabs to recruit participants and educate staff.
• Collaborate with clinicians to address individual needs.
