Qualifications

• Bachelor’s degree in social work, psychology, sociology, or human services; licensed if social work degree.
• At least one year of case management experience.
• Two years of experience with individuals with disabilities.
• Knowledge of community resources and services.
• Ability to work independently and manage stress.

Responsibilities

• Meet with individuals on caseload to assess needs.
• Coordinate services to meet wants and needs.
• Maintain confidentiality of records and interactions.
• Document intake and ongoing paperwork accurately.
• Develop and maintain community relationships.
