Job summary

MassAbility seeks a Waiver Program Coordinator in Fall River to oversee needs assessment, ISP development, service authorization activities, and implementation and monitoring of ISP for ABI-N and MFP-CL waivers statewide and for MFP Demonstration participants.

The role requires statewide travel to enroll participants, coordinating an interdisciplinary team, and ensuring timely completion of home accessibility modifications and linkage with LTSS and medical providers.

Qualifications

• Ability to travel statewide is required.
• Excellent communication skills in oral and written form, solid follow-through, flexibility.
• Ability to clarify and resolve problems and conflicts, and the ability to self-organize and manage priorities.
• Ability to work with remote technology (computer, VPN, remote calling) is required.
• Familiarity with needs of individuals with disabilities living in the community is a must.

Responsibilities

• Engage enrolled participants and families in person-centered planning to develop ISP.
• Meet in-person to perform or arrange necessary assessments pre-discharge.
• Develop a PCP for enrollees including services and community resources.
• Coordinate an interdisciplinary team to develop each Person-Centered Plan.
• Update assessments as necessary and arrange for special assessments.
• Safeguard waiver services and ensure delivery and satisfaction.
• Arrange referrals and monitor LTSS and linkage with medical providers.
• Conduct outreach to recruit participants and educate facility staff about waivers.
• Collaborate with clinicians to address individualized needs.
