Job summary

River Valley Behavioral Health is seeking a Waiver Care Manager to coordinate care, advocate for clients, and support families across multiple service sites. The role involves analyzing psychosocial needs, facilitating team-based treatment plans, and ensuring rights and safety are maintained.

You will complete essential trainings and participate in ongoing professional development to support individuals with developmental disabilities through a client-centered approach.

Qualifications

• BS/BA degree in Psychology, Sociology or Social Work or other human services field as approved by DDID.
• 1 year experience working with individuals with intellectual and/or developmental disabilities.
• Valid driver's license and ability to provide safe transportation.
• Excellent oral and written communication skills.

Responsibilities

• Analyzes client/guardian situations and compiles data to assess psychosocial needs.
• Maintains contact with service providers and professionals involved in the treatment plan.
• Identifies unmet needs and advocates for client/families regarding services and funding.
• Leads team meetings to develop, modify, and review treatment/service plans.
• Completes monthly summaries and maintains accurate records for goals and outcomes.
• Provides on-call support to assess risk and direct participants to appropriate resources.
• Trains direct support staff to deliver individualized supports across sites.
