Columbia River Mental Health Services is seeking a Care Coordinator to join our team in the SW Washington area. You will work with Health Homes and Health Plans to deliver community-based services for individuals with chronic health conditions, coordinating care to improve outcomes.

You will conduct outreach, screening, patient navigation, education on self-management, and coordinate referrals and transportation, while supporting discharge planning and collaboration with community partners.

Qualificações

• 1–2 years of experience in the social service field with mental illness or chemical dependency preferred.
• Strong communication and interpersonal skills.
• Ability to manage multiple tasks and deadlines.
• Flexible and open to change.
• Competent in basic computer literacy.

Responsabilidades

• Care coordination with health homes and plans to improve health outcomes.
• Initial contact and screening of referred individuals.
• Assist participants on navigating the healthcare system.
• Educate on self-management health tools.
• Coach participant/family to understand the Health Action Plan.
• Arrange transportation and coordinate referrals.
• Liaise with discharge planners and support post-discharge planning.
