Job summary

Tri-County Human Services seeks a dedicated professional to support individuals in navigating behavioral health services. You will liaise with multiple departments, coordinate care, and assist with applications to treatment programs while protecting client information.

The role emphasizes collaboration, timely follow-up, and engagement with clients to facilitate recovery, with a focus on dignity, respect, and cultural sensitivity in all interactions.

Qualifications

• Bachelor’s Degree required in a field related to Human Services; two years full-time mental health/addictions experience preferred.
• Computer experience/knowledge preferred.
• Staff training program participation is required.

Responsibilities

• Serve as liaison and advocate between CF BHN, Florida Center’s Admission Department, Primary Counselors, Probation, Referrals, and Aftercare Counselors.
• Attend Morning Report Monday through Friday and participate in 30-day reviews.
• Assist individuals and families through the application process to treatment programs.
• Coordinate sharing of protected information with providers/referrals to meet needs.
• Target resources and interventions to assist applicants and those seeking recovery.
• Maintain regular contact with clients to track progress and provide guidance and encouragement.
