Job description

VVFS Resource Specialist

The Virginia Department of Veteran Services is seeking a Resource Specialist (RS) to identify the behavioral health, rehabilitative, and other service needs of service members, veterans and/or their dependents/family members (SMVF). Based on those identified needs, the RS develops a coordinated services plan, connects the individual with appropriate service providers, offers care coordination support/assistance, and monitors for successful resolution of the identified needs.

Reporting & Guidance

The RS will report to the applicable VVFS Regional Director and performs duties directed by the VVFS SOP and under the guidance of the applicable VVFS Regional Director.

Responsibilities

  • Develop coordinated services plans based on identified needs.
  • Connect individuals with appropriate service providers.
  • Offer care coordination support and assistance.
