Responsibilities

• Lead the department's strategic Playbook and drive execution.
• Partner across departments to expand population health initiatives beyond traditional settings.
• Negotiate risk contracts, price unification, and major cyclical agreements.
• Develop PCP and specialty networks to expand access for underserved residents.
• Collaborate with FQHCs and community clinics to extend the primary care footprint.
• Guide risk stratification and reduce avoidable ED visits and readmissions.
• Foster data-driven decision-making and analytics for performance tracking.
