Job summary

You will assess hopes and desires, promote rights and advocacy, and coordinate supports across community and work settings while maintaining documentation and meeting training requirements. A valid driver's license and HS diploma are required.

Qualifications

• High School Diploma is required.
• Valid Driver's License (USA) is required.
• Experience with adults with mental health diagnoses and/or developmental disabilities is not required.

Responsibilities

• Assess individuals to identify hopes, dreams, and desired supports.
• Protect and promote the rights of individuals; assist with advocacy.
• Serve as a resource for services and supports meeting identified needs.
• Supervise, educate, and monitor individuals in work settings and day programs.
• Maintain a safe working environment by following safety procedures.
• Document progress notes, goals, incidents, timesheets, and required records.
• Attend meetings and trainings; maintain agency and state certifications.
• Travel and driving may be required.
• Sleepovers at residential settings may be needed.
