Volunteers of America Puerto Rico (VOAPR) is expanding its community health work by hosting an AmeriCorps VISTA member to develop sustainable systems that improve access to health and social resources for underserved populations.

The VISTA will help design a Community Health Needs Assessment, establish a Community Health Advisory Board, and strengthen connections among VOAPR, healthcare providers, social service organizations, and the communities served.

Responsibilities

Research community health needs and barriers to care affecting VOAPR populations.
Develop assessment tools and contribute to a Community Health Needs Assessment.
Build structure and operating tools for a Community Health Advisory Board.
Identify and engage healthcare providers, community organizations, and social service partners.
Develop a community resource directory, partnership tracking tools, and standardized referral processes.
Gather stakeholder feedback and prepare tools for continued implementation and expansion.

Job description

Volunteers of America Puerto Rico (VOAPR) is strengthening its community health infrastructure to improve access to health and social resources for underserved populations, including low-income older adults and individuals facing barriers to care. Building on VOAPR’s Community Health Worker model, AmeriCorps VISTA members will help create sustainable systems that expand community engagement, partnerships, referrals, training, data collection, volunteer involvement, and long-term program capacity.The Community Health Engagement & Partnerships VISTA will focus on understanding community health needs and strengthening connections among VOAPR, healthcare providers, social service organizations, community partners, and the populations we serve. The member will help develop a Community Health Needs Assessment, Community Health Advisory Board, community resource network, and standardized partnership and referral infrastructure.This is a capacity-building position. The VISTA will develop systems and resources that VOAPR and its Community Health Workers can continue using beyond the member's year of service.

Member Duties

Research community health needs, social determinants of health, existing resources, and barriers to care affecting populations served by VOAPR.
Develop community assessment tools and contribute to a Community Health Needs Assessment.
Build the structure and operating tools for a Community Health Advisory Board.
Identify and engage healthcare providers, community organizations, universities, government resources, and social service partners.
Develop a community resource directory, partnership tracking tools, and standardized referral processes.
Gather stakeholder feedback, evaluate systems developed during the service year, document recommendations, and prepare tools for continued implementation and expansion.
