Requirements include NYS RN license, a valid driver’s license with reliable transportation, and CHHA/home care experience. The position emphasizes patient quality of life and community education to reduce hospitalizations.

Qualifications

• NYS RN license required.
• Valid driver’s license and reliable transportation.
• Excellent clinical skills.
• Basic knowledge of office applications (Outlook, Word, printing and scanning).
• Home Care (CHHA) experience required.

Responsibilities

• Work closely with patients and families to provide home health care and support.
• Educate community and help prevent hospitalizations.
• Independently manage visits with office support as needed.
