Job summary

Americare is seeking a Visiting Nurse (Fee for Service RN) to join our team of Home Health Nurses. Based on availability, you will enjoy a flexible schedule with no mandatory night shifts, and the autonomy to manage your own time with office support when needed.

You will work closely with patients and families, educate your community, prevent hospitalizations, and focus on long‑term recovery and quality of life. NYS RN license, valid driver’s license, and CHHA/home care experience are required.

Qualifications

• NYS RN License required.
• Valid driver’s license and reliable transportation.
• Strong ability to work independently with minimal supervision.
• Excellent clinical skills.
• Home Care (CHHA) experience required.
• Basic knowledge of office applications (Outlook, Word, printing and scanning applications).

Responsibilities

• Provide in-home nursing care to clients.
• Educate patients and families to prevent hospitalizations.
• Manage and coordinate care with office support when needed.
