Job summary

Americare is seeking a Visiting Nurse (RN) to provide compassionate home health care with flexible scheduling. The role emphasizes independence, patient education, and reducing hospitalizations for better quality of life.

You will operate with office support when needed, serving patients and families in their homes while maintaining high clinical standards and autonomy in decision-making.

Qualifications

• NYS RN License is required.
• Valid driver’s license and reliable transportation.
• Ability to work independently with minimal supervision.
• Excellent clinical skills.
• Home Care (CHHA) experience required.

Responsibilities

• Work closely with patients and families, educate the community and prevent hospitalizations.
• Provide visiting nurse home health care with autonomy and accountability.
• Support long‑term recovery and improve quality of life for patients.
