Job summary

Colorado Coalition for the Homeless is seeking a Patient Navigation Specialist- Vision to help patients access comprehensive eye care and overcome barriers. You will work at the Stout Street Health Center in Denver, serving as the front desk liaison and coordinating referrals and appointments.

Qualifications

• High School diploma or equivalent required.
• One year of experience in a healthcare setting, preferably in eye care.
• Experience with insurance enrollment and front desk operations preferred.

Responsibilities

• Act as first point of contact for patients with a trauma-informed, culturally responsive approach.
• Register clients, verify insurance coverage, create health records, and schedule appointments in the EHR.
• Verify insurance status and connect patients to enrollment resources.
• Assist with scheduling eye exams and referrals, and arrange transportation.
• Support clinic operations and document patient follow-up in clinic workflows.
• Perform optical-related duties such as eyeglass adjustments and entering orders.
