Job summary

The Colorado Coalition for the Homeless in Denver is seeking an On-Call Patient Navigation Specialist for our eye clinic. This front desk role helps patients access care, schedule appointments, and navigate insurance and transportation barriers.

You will assist ophthalmic staff with basic vision services, coordinate referrals, and maintain accurate patient records, while upholding a trauma-informed, confidential approach.

Qualifications

• High school diploma or equivalent required.
• Minimum 1 year in a healthcare setting preferred (eye care preferred).
• Experience with insurance enrollment and front desk operations preferred.
• Strong customer service and communication skills.

Responsibilities

• First point of contact for patients, ensuring a trauma-informed, professional experience.
• Register patients, verify insurance, create health records, and input data into the EHR.
• Verify insurance status and connect clients to enrollment resources.
• Schedule eye exams and referrals; arrange transportation for appointments.
• Escort patients to specialty referrals for procedures as needed.
• Assist the Ophthalmic Assistant with screenings and basic eye-care services.
• Handle optical duties: eyeglasses adjustments, fittings, and order entry.
• Coordinate care with providers and document follow-up in workflows.
• Perform other duties as assigned.
