Job summary

The Colorado Coalition for the Homeless seeks an On-Call Patient Navigation Specialist - Vision to support our eye clinic in Denver. This frontline role helps patients obtain and keep specialty appointments at external providers and includes basic vision screenings to assist triage and care planning.

The position focuses on removing barriers related to transportation and referrals, while upholding trauma-informed, patient-centered care.

Qualifications

• Front desk or patient navigation experience preferred.
• Possible interaction with external organizations for referrals.
• Ability to work in a trauma-informed, patient-centered environment.

Responsibilities

• Serve at the front desk for the eye clinic and assist patients with appointments at outside organizations.
• Perform basic vision screenings to triage patient care.
• Address transportation barriers and health education needs to support referrals.
• Support trauma-informed, patient-centered service delivery.

The On-Call Patient Navigation Specialist- Vision

This position serves in the front desk role for our eye clinic, assists patients in obtaining and keeping specialty appointments for care at outside organizations, and performs basic vision screenings to help triage patient care. The role contributes to CCH's mission by addressing barriers related to transportation, specialty referrals, and health education while fostering a trauma-informed, patient-centered approach.

Location: 2130 Stout Street, Denver, CO (Stout Street Clinic)

Schedule: 7:30am - 4pm
