Devoted Health is expanding its TOC program with Advanced Practice Providers (APRNs and PAs) to deliver virtual, post‑discharge care for geriatric patients. You will assess, reconcile medications, order tests, and educate families during the crucial 30‑day window after hospitalization.

You will work within an interdisciplinary team including RNs, care coordinators, social workers, pharmacists, and PCPs, contributing to program growth and EHR enhancements.

Qualifikationen

• Minimum 5+ years direct patient care in geriatric populations.
• Experience with post-discharge care and telehealth visits.
• Active medical licenses (RN/APRNs or PA) in good standing.

Aufgaben

• Conduct thorough post-discharge assessments and medication reconciliation.
• Place orders (prescriptions, labs, referrals) and provide education.
• Perform acute/virtual urgent-care TOC visits and determine level of care.
• Adjust plans of care and participate in interdisciplinary TOC rounds.
• Contribute to quality-improvement projects and provide EHR feedback.
