Job summary

Devoted Health is seeking an experienced Advanced Practice Provider (APRNs or PAs) to lead our Telehealth‑based Transitions of Care program. You will perform comprehensive initial post‑discharge visits, conduct acute/virtual visits, and shape a patient‑centered chronic care plan over 30 days.

You will collaborate with nurses, social workers, pharmacists and PCPs, participate in TOC rounds, and help build the program and EHRs in a fast‑paced environment where change is constant.

Qualifications

• 5+ years direct patient care in geriatric populations with multiple chronic conditions across acute and chronic presentations.
• Active RN/APRN or PA license; able to obtain required state licenses within 90 days of hire.

Responsibilities

• Perform comprehensive initial TOC visits including post‑discharge assessments, medication reconciliation, orders, and care planning.
• Conduct acute/virtual TOC visits to diagnose and manage new or worsening symptoms in complex patients.
• Manage the 30‑day trajectory by adjusting chronic disease plans and coordinating care.
• Participate in interdisciplinary TOC rounds and collaborate with care managers, social workers, pharmacy, and PCPs.
• Support build of the TOC program, contribute to quality improvement, and provide feedback on the EHR.
• Be adaptable to evolving service needs and participate in broader organizational initiatives.
