Job summary

Devoted Health is seeking an Advanced Practice Provider (APRNs or PAs) to lead our Medical Transitions of Care program. You will perform post-discharge assessments, reconcile medications, place orders, and conduct acute/virtual TOC visits to prevent readmissions.

You’ll shape the program, collaborate with nurses, social workers, pharmacists, and physicians, and help evolve our homegrown EHR while delivering high-quality, patient-centered care.

Qualifications

• 5+ years of direct patient care with geriatric, complex chronic conditions.
• Full-time availability as described (Mon–Fri, 8:30 AM–5:00 PM).
• Experience performing telehealth visits for post-discharge care.
• Experience managing acute/chronic disease exacerbations (CHF, diabetes, COPD, hypertensive emergencies).
• Licensure in good standing as APRN or PA.

Responsibilities

• Conduct post-discharge assessments of medical conditions, meds, function, and psychosocial needs.
• Complete medication reconciliation and optimize guideline-directed therapy.
• Place orders, provide education, and communicate contingency plans and red flags.
• Perform acute/virtual TOC visits and determine the appropriate level of care.
• Adjust care plans responsively as the clinical picture evolves.
• Participate in interdisciplinary TOC rounds and collaborate with care teams.
• Contribute to clinical education, quality-improvement projects, and EHR feedback.
