Job summary

Devoted Health is seeking experienced Advanced Practice Providers (APRNs and PAs) to join our Medical Transitions of Care (TOC) program. The role focuses on virtual post-discharge care within a 30-day window to prevent readmissions, collaborating with an interdisciplinary team including care coordinators, nurses, social workers, and physicians.

Roughly 70% of time is spent on initial post-discharge visits and 30% on follow-up and acute visits.

Qualifications

• 5+ years of direct patient care managing geriatric populations with multiple chronic, complex conditions across acute and chronic presentations.
• Full-time, five 8-hour days/week (Mon–Fri, 8:30 AM–5:00 PM) with active RN/APRN or PA licensure.
• Experience performing visits over telehealth video platforms.

Responsibilities

• Conduct comprehensive post-discharge assessments with medications, orders, education, and contingency plans.
• Perform acute/virtual TOC visits and determine level of care, with bias to action.
• Reinforce and adjust care plans as clinical picture evolves.
• Participate in interdisciplinary TOC rounds and collaborate with care team and PCPs.
