Job summary

Prescribe FIT is seeking a Health Coach Care Coordinator to engage with patients virtually via our software, educate them on disease processes, self-management, lifestyle changes, and motivate ongoing wellness. The role emphasizes empathy, problem-solving, and proactive communication with patients and care teams.

Responsibilities include charting interventions, responding to patient messages by digital and phone channels, and providing personalized coaching on activity, nutrition, and lifestyle

Qualifications

• Gainfully employed for 2+ years as a Medical Assistant or other healthcare professional working directly with patients.
• Associates degree or higher in Medical Assisting or healthcare education is preferred.
• Current nationally recognized certification or willingness to renew is preferred.

Responsibilities

• Chart and document patient interventions, provider interactions, and clinical notes.
• Be responsive to patient communications via digital, phone, video, and text.
• Provide thorough, personalized patient support and education on activity, nutrition, and lifestyle changes.
• Engage with patients routinely via virtual or telephonic methods.
• Use motivational interviewing to address issues and support behavior change.
