Job summary

Responsibilities include outreach via phone, email, text, and coaching conversations, using motivational interviewing to empower self-management of chronic conditions, documenting cases, and supporting special projects. The role offers competitive hourly compensation and comprehensive benefits.

Qualifications

• Bachelor’s Degree in relevant discipline or MA/EMT/Paramedic certification.
• 2+ years experience in a healthcare delivery or related environment.
• Excellent written and verbal communication skills.

Responsibilities

• Conduct member outreach to drive behavior change and positively impact clinical outcomes.
• Engage with members via phone, email, text, virtual application, and coaching conversations to provide feedback and support.
• Utilize a coaching approach congruent with motivational Interviewing to empower self-management of chronic conditions.
• Refer members for support and education for conditions as needed based on clinical care guidelines.
• Maintain accurate and thorough case documentation to Teladoc Health standards.
• Complete administrative duties essential for smooth operation.
• Support corporate initiatives and special projects as needed (5%).
