Cvausa! is seeking a dedicated Health Coach to support patients in managing heart failure through education and behavior change. This full-time, remote role involves partnering with clinical teams to enhance patient care and engagement.

The ideal candidate will have a bachelor’s degree in health sciences and NBC-HWC certification. You will provide education on lifestyle modifications and assist patients in achieving their health goals while maintaining effective communication.

Qualifications

• Bilingual proficiency in English and Spanish preferred.
• Experience in telehealth or remote patient engagement preferred.

Responsibilities
