Job summary

Mercy Community Health is seeking a dedicated Community Health Worker I to serve as a trusted liaison between patients, healthcare providers, and community resources in a virtual outpatient setting. You will educate patients on coverage options, assist with applications, and connect them to services while navigating barriers to care.

You will advocate for patients, coordinate resources, document encounters in the EMR, manage a caseload, and pursue ongoing professional development through Mercy

Qualifications

• High School Diploma or equivalent is required.
• One year of Community Health Worker experience or related field.
• Completion of Mercy-provided CHW Training Program within 6-12 months of hire.

Responsibilities

• Patient Advocacy & Navigation: guide patients through healthcare and human service systems to access care and support services.
• Community Resource Coordination: connect patients with appropriate community resources and maintain relationships with providers.
• Documentation & Reporting: document patient interactions in the EMR and track program metrics.
• Patient Outreach & Caseload Management: manage a caseload and conduct outreach via calls and secure messaging.
• Professional Development: participate in CHW training and continuing education.
