Job summary

Mercyhealth Community Health Worker role provides culturally appropriate health education and outreach in community settings, including homes, clinics and shelters, coordinating with the patient’s healthcare team to meet care needs and ensure the right level of service.

You will assess wellbeing, collaborate with patients and resources, and document interactions while supporting patients in achieving goals and improving health outcomes.

Qualifications

• High school diploma or equivalent.
• Strong interpersonal and communication skills.
• Knowledge of Social Determinants of Health barriers.
• Active listening and ability to develop trusting relationships.
• Creative thinking.

Responsibilities

• Assess wellbeing, living environment and safety.
• Communicate with patients, community resources, and healthcare team(s).
• Provide individualized education to patients—including symptoms, treatment options, medication management and lifestyle modifications.
• Assist patients with individually set goals, develop action plans and provide ongoing support to promote behavior change and self-efficacy.
• Document all patient interactions.
• Serve as a patient advocate, helping navigate the healthcare system, access resources and overcome barriers to healthcare.
• Accompany patients to medical appointments, assist with forms and facilitate communication between patient and provider.
• Provide emotional and social support to patients, helping them cope with living with chronic conditions.
