Responsibilities

• Serve as a health navigator for uninsured and underinsured patients.
• Provide one-on-one support to high-risk patients to overcome barriers to care.
• Assist with Medicaid and financial assistance applications.
• Collaborate with Eligibility Services to gather documentation and streamline processes.
• Connect patients with primary care providers and medical homes.
• Document patient encounters and referrals in the EMR.
