Job summary

Mercy seeks a Community Health Worker II to serve as a bridge between patients, care teams, and community resources. You will educate, advocate, and follow up to address health-related social needs and improve access to care.

You will document in the EMR, coordinate referrals, assist with healthcare coverage applications, and collaborate with a multidisciplinary team to reduce barriers to services. This role requires skills in communication and compassion for underserved populations, with travel

Qualifications

• High School Diploma or equivalent required.
• 1-3 years of Community Health Worker experience or equivalent relevant experience.
• Mercy-provided CHW training certificate within 6-12 months of hire.
• Basic Life Support (BLS) certification.

Responsibilities

• Work one-on-one with patients identified through referrals from care teams and those screening positive for health-related social needs.
• Educate patients on healthcare and social service programs and assist with coverage and financial aid applications.
• Coordinate access to community resources and maintain collaborative relationships with providers.
• Document patient interactions in the EMR and track outcomes.
• Maintain an appropriate patient caseload and conduct follow-up via calls, patient portals, and other approved platforms.
• Participate in ongoing CHW training and complete Mercy's certification within required timeframes.
• Perform other duties as assigned.
