Job summary

MUSC Health is seeking a Virtual Care Access Concierge – Registered Nurse (RN) to ensure seamless handoffs from virtual visits and timely escalation when needed. The role collaborates with multidisciplinary teams to reduce friction in care transitions while preserving clinical information continuity.

Under supervision, you will advise on scheduling, coordinate follow-ups, and support patients and families through theAfter Visit Summary process, with emphasis on patient advocacy and care

Qualifications

• RN licensure in South Carolina or a compact state.
• BSN preferred; ADN allowed with BSN enrollment within two years.
• Current BLS certification required.

Responsibilities

• Use the nursing process to liaison with patients and caregivers for scheduling within the complex system.
• Advocate for patients and families with a compassionate approach.
• Coordinate discharge plans and reminders, scheduling follow-ups and warm hand-offs.
• Liaise with ambulatory clinics and virtual services to improve hand-offs.
• Apply clinical judgement and evidence-based info to manage care transitions.
• Educate patients/families about After Visit Summary goals.
• Identify problems and utilize resources to address them.
• Document orders, care plans, and interactions in the EMR.
