Job summary

The Medical University of South Carolina seeks a Virtual Care Access Concierge – Registered Nurse (RN) to ensure seamless handoffs from virtual visits and coordinate care transitions across teams. Qualifications

• Bachelor of Nursing degree preferred; RN licensed in SC or a compact state.
• Minimum of one year of RN experience.
• Current BLS certification required.

Responsibilities

• Utilize the nursing process to serve as a patient liaison to assist patients and caregivers in scheduling within the complex healthcare system.
• Implement discharge plans and coordinate patient and clinical reminders, scheduling follow-up visits, and ensure warm hand-off of patient and clinical context to the next level of care.
• Liaison with ambulatory clinics and virtual services to improve hand-off and scheduling for post-virtual care follow-up needs.
• Accurately document all orders, care plans and interactions in the EMR to maintain current patient records.
