Qualifications

Responsibilities

• Serve as patient liaison to schedule within the healthcare system.
• Advocate for patients and families with a compassionate approach.
• Implement discharge plans and coordinate follow-up and warm hand-off.
• Coordinate care transitions with ambulatory clinics and virtual services.
• Apply clinical judgment and evidence-based updates to care transitions.
• Educate patients and families about After Visit Summary goals.
• Document orders and care plans in the EMR to maintain records.
