Job summary

Medical University Hospital Authority (MUHA) is seeking a Virtual Care Access Concierge – Registered Nurse (RN) to support telehealth transitions. You will function as a patient liaison, ensure smooth handoffs, and coordinate follow-up care within the ambulatory and virtual services network.

Responsibilities include using the nursing process, advocating for patients, coordinating discharge plans, and documenting in the EMR to maintain continuity of care.

Qualifications

• Bachelor of Nursing degree preferred.
• RN license in SC or a compact state required; BSN enrollment within two years if not yet BSN.
• Current BLS certification (AHA) or Red Cross BLS for healthcare providers required.
• Minimum of one year of RN experience required.

Responsibilities

• Utilize the nursing process to serve as a patient liaison to assist scheduling within the healthcare system.
• Act as a patient/family advocate with a compassionate approach.
• Implement discharge plans and coordinate follow-up visits and warm hand-offs of clinical context.
• Liaise with ambulatory clinics and virtual services to improve hand-off and scheduling for post-virtual care follow-ups.
• Coordinate care transitions using analytic reasoning and evidence-based practice; document all orders in EMR.
