Job summary

CENTER FOR VETERANS ISSUES INC in Marathon, WI seeks a full-time Case Manager to provide crisis intervention, risk assessment using C-SSRS, WRAPs and safety planning, and comprehensive support to Veterans.

You will conduct assessments, develop ISPs, coordinate with VA resources, and advocate for trauma-informed services while building community partnerships. Requires 2+ years in case management and a 4-year degree.

Qualifications

• Minimum two years of experience in case management including needs assessment and service planning.
• Experience working with Veterans strongly preferred.
• Experience delivering services to individuals with complex and intensive needs, including mental disorders and/or substance abuse, strongly preferred.
• Resident of the proposed service area with familiarity with the region and resources preferred.
• Valid Wisconsin driver's license and auto insurance in good standing.

Responsibilities

• Ensure the immediate safety of all individuals being assessed through active crisis management and de-escalation.
• Administer the C-SSRS to evaluate risk and determine appropriate interventions.
• Develop WRAPs and safety plans for participants.
• Provide gatekeeper training to veterans, family members, and partners.
• Conduct mental health screenings and needs assessments.
• Develop ISPs that promote recovery and personal growth.
• Monitor progress and adjust goals and supports as needed.
• Refer participants to VA resources, housing, and employment services.
• Assist with scheduling initial appointments and follow-ups.
• Advocate for participants' rights; ensure trauma-informed, respectful care.
• Conduct life-skills education and budgetary literacy sessions.
• Maintain documentation per program standards; ensure compliance with regulations.
• Participate in staff meetings and training; support program evaluations.
• Represent program at community events and maintain referral relationships.
• Other duties as assigned.
