Responsibilities

• Ensure the immediate safety of all individuals through crisis management and de-escalation.
• Administer the C-SSRS to evaluate risk and determine interventions.
• Develop WRAPs and safety plans tailored to each participant.
• Provide gatekeeper training to Veterans, family members, and community partners.
• Conduct mental health screenings and needs assessments.
• Develop ISPs that promote recovery, self-determination, and growth.
• Refer participants to VA resources for housing, behavioral health and employment services.
• Assist participants with scheduling and attending initial appointments, with follow-up.
• Advocate for participants' rights, ensuring equitable, trauma-informed services.
• Conduct life skills education including budgeting and coping strategies.
• Maintain and strengthen relationships with referral sources and represent the program publicly.
• Maintain confidential documentation and ensure compliance with policies and regulations.
