Job summary

Community Counseling Center Inc in Cape Girardeau, MO seeks a Veterans Liaison to connect Veterans, Service Members, and families with suicide prevention, behavioral health, and supportive services. You will lead outreach, screen for risk, perform intake assessments, and coordinate care using trauma-informed, person-centered approaches.

Qualifications include a Bachelor's degree in a social service field and at least one year in behavioral health or related settings.

Responsibilities

• Conduct community outreach and build referral relationships with VA facilities and local organizations.
• Screen participants for eligibility and suicide risk using approved tools and agency protocols; complete intake assessments.
• Develop service plans, provide case management, and monitor participant progress.
• Coordinate referrals and warm hand-offs for behavioral health care, housing, benefits, transportation, employment, and other needs.
• Respond to crises according to agency procedures and coordinate emergency referrals when needed.
• Maintain timely documentation, participate in required reporting and training, and protect participant confidentiality.
• Provide ongoing case management services, including monitoring participant progress and coordinating referrals.
• Support outreach events and comply with agency policies, grant requirements, and applicable regulations.

Schedule:

Monday - Friday 8:00am - 4:30pm (Times may vary; occasional evening or weekend maybe required)

Location:

Cape Girardeau, MO (Cape County), serve all five counties which include Cape, Fredericktown, Marble Hill, Perryville, and Ste Genevieve Missouri.

Pay Non - Exempt:

$20.00 - $22.40 per hour (Depending on experience)

Job Summary:

The Veterans Liaison connects Veterans, Service Members, and eligible family members with suicide prevention, behavioral health, and supportive services. This position provides outreach, screening, case management, and care coordination using a trauma - informed, person - centered approach.

Minimal Requirements:

• Bachelor's degree in Social Work, Psychology, Sociology, Human Services, Counseling, or related social services field required.


• Minimum of one (1) year experience working in behavioral health, social services, case management, Veteran services, crisis intervention, or related field desired.


• Knowledge of community resources, behavioral health systems, and supportive service programs.


• Strong communication, organizational skills, and documentation skills; ability to maintain professional boundaries and confidentiality.


• Ability to work effectively with diverse populations using a trauma-informed and culturally responsive approach.


• Ability to work with diverse population using a trauma-informed approach.


• Experience working with Veterans or knowledge of military culture desired.


• Must hold Valid Driver's License (Class E for Missouri or Class D for IL Residents - Obtain on start date) and current continuous auto insurance. Ability to travel within the service area.
