Job summary

Community Counseling Center provides community support services for assigned clients within the CPRC. The role involves coordinating delivery of mental health services, assessing client adjustment, linking with community resources, and providing crisis response to reduce hospitalization risk.

Typically manages 20–25 clients, meeting weekly at homes or residential centers. The position emphasizes delivering structured support, maintaining treatment goals, and documenting progress in alignment

Qualifications

• Qualifying experience must include delivery of service to individuals with mental illness, substance abuse disorders, or developmental disabilities.

Responsibilities

• Provide community support services to children and adult clients enrolled in CPRC.
• Maintain a caseload as assigned including in the community and during hospitalizations.
• Maintain direct services productivity in accordance with assigned caseloads.
• Maintain positive attitude toward agency and clientele.
• Maintain appropriate boundaries with clients and staff members.
• Provide training/education to clients in daily living skills.
• Monitor completion of treatment plan goals and objectives and maintain contact with treatment team in regard to progress and any concerns.
• Refer to appropriate resources within the community to assist clients in maintaining/improving their functionality and to develop a personal support system.
• Responsible for transporting clients in agency and/or personal vehicle as deemed necessary and appropriate to meet program requirements.
• Successfully complete DMH approved Wellness Coaching training through the Relias Learning platform.
• Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per agency standards.

Education

Bachelor's degree or higher

Job description

Location: Cape Girardeau, MO - Cape Girardeau County

Full Time: Non - Exempt

Work Hours: Monday - Friday 8:00am - 4:00pm, Some evenings maybe required as necessary dependent upon agency and client need.

Pay Range: $20.00 - $22.40 per hour

Note: Pay will vary dependent upon prior experience.

Qualifications:

• Bachelor's degree or higher from accredited university; or
• Any four-year combination of higher education and qualifying experience; or
• Four years of qualifying experience.
• Valid Drivers License - Must hold or obtain by start date, MO Class E Driver's License (Class D for IL Residents) and current auto insurance.
• Travel in own vehicle is required.
