Job summary

Community Counseling Center Inc in Cape Girardeau, MO, seeks a CPS/CPRC professional to deliver community support services and coordinate mental health resources. Responsibilities

• Provide community support services to CPRC clients in homes or residential centers.
• Maintain a caseload across community settings and during hospitalizations.
• Maintain direct services productivity per caseloads.
• Engage with clients and staff professionally while upholding boundaries.
• Deliver training/education in daily living skills to clients.
• Monitor progress toward treatment plan goals and coordinate with the treatment team.
• Refer clients to community resources to sustain functioning and build support.
• Transport clients as needed using agency or personal vehicle.
• Complete DMH approved Wellness Coaching training via Relias Learning.
• Document according to clinical policies and program guidelines.

Location: Cape Girardeau, MO - Cape Girardeau County

Full Time: Non - Exempt

Work Hours: Monday - Friday 8:00am - 4:00pm, Some evenings maybe required as necessary dependent upon agency and client need.

Pay Range: $20.00 - $22.40 per hour

Note: Pay will vary dependent upon prior experience.

Qualifications:

• Bachelor's degree or higher from accredited university; or
• Any four-year combination of higher education and qualifying experience; or
• Four years of qualifying experience.
• Valid Drivers License - Must hold or obtain by start date, MO Class E Driver's License (Class D for IL Residents) and current auto insurance.
• Travel in own vehicle is required.

Job Summary:

Responsible for the delivery of community support services for assigned clients within the Community Psychiatric Rehabilitation Center (CPRC). These activities include coordination of delivery of mental health services, assessment and monitoring of client adjustment, resource development and linkage with community resources, providing crisis response to reduce risk of hospitalization of clients in crisis. Typically, the position manages 20 - 25 clients, meeting with them weekly at client homes or residential centers.

Job Duties:

1. Provide community support services to children and adult clients enrolled in CPRC program.
2. Maintain a caseload as assigned including in the community and during hospitalizations.
3. Maintain direct services productivity in accordance with assigned caseloads.
4. Maintain positive attitude toward agency and clientele.
5. Maintain appropriate boundaries with clients and staff members.
6. Provide training/education to clients in daily living skills.
7. Monitor completion of treatment plan goals and objectives and maintain contact with treatment team in regard to progress and any concerns.
8. Refer to appropriate resources within the community to assist clients in maintaining/improving their functionality and to develop a personal support system.
9. Responsible for transporting clients in agency and/or personal vehicle as deemed necessary and appropriate to meet program requirements.
10. Successfully complete DMH approved Wellness Coaching training through the Relias Learning platform.
11. Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per agency standards.
