Job summary

Veterans Bridge Home is seeking an Intake and Eligibility Coordinator- SSVF to determine program eligibility and coordinate services for Service Members, Veterans, and their families. The role interacts with the Market Executive Director, SSVF Program Manager, Healthcare Navigator, and Housing Navigation team.

You will assess eligibility based on Veteran status, income, housing status, and level of need, provide direct casework, and document in HMIS. Collaboration with HUD partners is essential.

Qualifications

• Bachelor's degree in social work or related field and prior case management experience.
• Experience with homelessness or housing instability and the ability to build rapport with people with complex needs.
• Knowledge of mental health challenges, co-occurring conditions, and substance use recovery.

Responsibilities

• Conduct participant interviews and assessments; verify Veteran status, housing status, income eligibility, and level of need.
• Provide ongoing case coordination and collaborate with the Healthcare Navigator and Housing Navigator.
• Develop, implement, monitor, and update individualized service plans.
• Connect participants with community resources and referrals.
• Conduct home visits as appropriate to assess needs and safety.
• Collect and upload eligibility documentation into HMIS and tracking systems.
• Complete required monthly reports and ensure compliance with funding/licensing standards.
• Maintain relationships with internal teams and external partners to coordinate services.
