Job summary

Catholic Community Services is looking for a Case Manager for the Veterans Services program. This role involves managing a caseload, conducting intakes, and developing Housing Stability Plans. Qualifications

• Experience in providing services to low-income/no-income households.
• Ability to work with a diverse spectrum of high-need individuals.
• Must have reliable transportation, valid driver’s license, and insurance.

Responsibilities

• Complete thorough intakes and eligibility screening with referred clients.
• Initiate Housing Stability Plan tailored to clients' needs.
• Provide data entry into the Homeless Management Information System.
• Update and carry out client Housing Stability Plans.
• Attend meetings and represent CCS Veteran Services Program.
