Job summary

Center For Veterans Issues Inc seeks a Case Manager to provide strengths-based services to homeless veterans in transitional housing. You will support housing stability, benefits access, and address barriers such as mental health or substance use, partnering with VA services to promote self-sufficiency.

Responsibilities include intake, ISP development, service coordination, HMIS documentation, crisis response, advocacy, and collaboration with clinical staff, community partners, and residents.

Qualifications

• Bachelor’s degree in social work, psychology, human services, or related field; Master’s preferred.
• Minimum of two years of case management experience; 3-5 preferred.
• Strong knowledge of veterans' issues, housing systems, mental health, and substance use recovery.
• Experience with homeless populations and/or supportive housing.
• Excellent interpersonal, crisis intervention, and documentation skills.

Responsibilities

• Conduct intake assessments to determine eligibility and needs.
• Develop, implement, and update ISPs with measurable housing and health goals.
• Coordinate internal and external services and appointments.
• Maintain accurate case notes and HMIS-compliant documentation.
• Respond to crises and connect residents with emergency services.
• Advocate for resident rights and access to resources.
• Collaborate in team meetings and with community partners.
