Job summary

Paycom - ATS is seeking a Case Manager to provide outreach and case management to Homeless Veterans under the GPD VA grant in Columbia, SC area. The role emphasizes housing, employment, finances, and thorough documentation.

The candidate should hold a Master’s in Social Work or related field, possess a valid driver’s license, and have experience with HUD/VASH. Collaboration with VA liaisons and community resources is key.

Qualifications

• Master’s degree in Social Work, Human Services or Sciences (or a related Bachelor’s with 3 years related experience).
• Valid Driver’s License.
• Proof of Auto Liability Insurance if required.
• Experience with homeless Veterans and HUD/VASH programs is preferred.

Responsibilities

• Provide case management for residents focusing on housing, employment, and finances.
• Maintain accurate charts and documentation for each resident.
• Develop initial treatment plans within 72 hours of admission and 90-day plan reviews.
• Prepare reports for partner agencies and coordinate with VA and community resources.
