Job summary

The Salvation Army's Alabama Corps—Tuscaloosa is seeking a Full-Time Pathway of Hope Case Manager to interview applicants, assess eligibility, and set individualized goals for eligible clients within a comprehensive service framework.

The role requires maintaining client records in HMIS, managing HUD goals, and providing case management to veterans; a Bachelor's degree and valid driver’s license are required, with regular office duties and field transport as needed.

Qualifications

• Bachelor's degree in Social Work, Behavioral Science, or related field.
• Three years progressively responsible experience providing direct case management services.
• Valid State Driver's License.

Responsibilities

• Interviews applicants to determine eligibility for Pathway of Hope program enrollment and conducts needs assessment.
• Establishes long-term program goals for eligible clients and counsels on modifying negative behaviors.
• Maintains case records and HMIS data entry; ensures accuracy of records.
