Job summary

Peninsula Agency on Aging is seeking an Options Counselor for the VDC Program to provide case management and service coordination for eligible veterans in MD, DC, and VA.

The role involves advising veterans and caregivers, conducting assessments, counseling on options, and developing comprehensive care plans; requires traveling, notary preferred, strong communication, and computer skills.

Qualifications

• Bachelor's degree in social work, human services, or related field.
• Experience with veterans and/or disabled adults; case management.
• Excellent interviewing, oral, and written communication; valid driver’s license.

Responsibilities

• Advise veterans and caregivers on services and resources for aging in place.
• Screen and complete assessments; provide options counseling.
• Develop comprehensive plans of care/service plans for veterans.
• Coordinate case management and service coordination across MD, DC, and VA.
