Job summary

Community Hope, Inc. is seeking a dedicated professional to support Veterans and their families through the Supportive Services for Veteran Families program.

The role involves screening, information, and referrals, with a focus on compassionate service delivery. The position requires a Bachelor’s degree in a related field and at least one year of human services experience, with a preference for call center or hotline background; hybrid schedule available with remote work one day per week after

Qualifications

• Bachelor’s degree in a relevant field required.
• 1 year of human service experience preferred.
• Call center or hotline experience preferred.

Responsibilities

• Perform initial eligibility screening via phone and email.
• Provide information and referral for mental health, housing, legal, and other services.
• Serve as first point of contact for veterans and families with excellent customer service.
• Create, maintain, and update a catalog of community resources.
• Document service delivery accurately in the electronic health record.
• Maintain confidentiality of participant information.
• Attend required training and supervision meetings.
• Collaborate with the SSVF team to ensure quality service delivery.
