This role emphasizes collaboration with supervisors and peers, adherence to policy, and community resource referrals, with occasional evening or weekend hours and transportation duties as needed.

Qualifications

• Able to read and comprehend simple instructions, short correspondence, and memos.
• Able to write simple correspondence and clinical documentation in medical records.
• Able to present information effectively in one-to-one and small group settings.
• Veteran or Military-Connected individual preferred.

Responsibilities

• Adheres to agency policies, CARF/DMHSAS standards, and confidentiality rules.
• Supports Veterans and Military-Connected clients with recovery-focused services.
• Establishes professional relationships with supervisor and peers; works in teams.
• Monitors eligibility and assists with Medicaid/VA benefits as eligible.
• Provides recovery-oriented services and helps clients access community resources.
• Maintains confidential documentation and completes records on service day.
• May work evenings or weekends and may provide client transportation.
• Performs other related duties as assigned.
