Job summary

Community Hope is seeking a dedicated case manager to support veterans in recovery, stability, and independence within a residential program. You will provide assessments, recovery planning, advocacy, and connections to community services, helping residents transition and plan next steps.

The role requires a Bachelor's in Social Work or related field, at least one year of case management experience, and a valid driver's license.

Qualifications

• Bachelor’s Degree in Social Work, Psychology or a related field.
• Minimum 1 year of case management experience.
• A valid driver’s license in good standing.

Responsibilities

• Provide case management, assessments, recovery planning, advocacy, and connections to community services.
• Help veterans transition into the residence and prepare for discharge or placement in other housing.
• Facilitate recovery support groups, recreational activities, and monthly veteran meetings.
• Provide medication education and conduct on-site drug and alcohol screenings when needed.
• Assist with staff scheduling, program coverage, and emergency shift coverage.
• Help maintain a safe, clean residence and support veterans with housekeeping and facility upkeep.
• Maintain accurate, timely service records and follow agency fiscal and billing procedures.
• Transport veterans in agency vehicles when needed and work with your team to cover shifts and program responsibilities.
