Job summary

Qualifications

• 2+ years of experience in case management or behavioral health.
• Experience working with high-risk populations preferred.
• Experience in trauma-informed care and suicide prevention.

Responsibilities

• Conduct thorough assessments to identify participant needs.
• Develop individualized plans to reduce suicide risk.
• Coordinate services for mental health and social support.
• Maintain documentation per grant requirements.
• Participate in team meetings and case conferences.
