Job summary

Volunteers of America Chesapeake & Carolinas is seeking a Case Manager to provide day-to-day support for veteran households under the SSVF program in NC. The role focuses on homelessness prevention, rapid re-housing, and wraparound services, coordinating with VA, housing providers, and community partners to promote housing stability.

You will conduct intake, assessments, and service planning, and advocate for clients while maintaining confidentiality and documentation.

Qualifications

• Bachelor’s degree preferred (counseling psychology, rehabilitative counseling, or related field) or comparable experience.
• Experience working with homeless individuals and families including veterans is a plus.

Responsibilities

• Coordinate outreach efforts for the SSVF program and establish a plan to reach veterans and families.
• Screen potential participants for program eligibility and verify information with applicable agencies.
• Conduct housing and comprehensive supportive services assessments and develop a Family Services Plan.
• Provide service coordination between the program and community partners.
• Develop placement, stabilization and follow-up plans with clients, family members and housing partners.
• Arrange for housing support and wraparound services to ensure transition to permanent housing.

Education

Bachelor's degree in counseling psychology, rehabilitative counseling, or related human service field
