Job summary

Minnesota Assistance Council for Veterans (MACV) in Moorhead is seeking an intensive case manager to support Veterans and families facing homelessness. You will assess financial, personal, and crisis needs, and develop housing stability plans with clients.

The role coordinates with staff and community partners across Moorhead, Bemidji, and St. Cloud, delivering mobile services as needed and utilizing a comprehensive database. A related degree helps, with strong communication and organization.)

Qualifications

• Strong communication with Veterans and providers.
• Ability to solve problems and resolve crises under pressure.
• Excellent organizational and time-management skills across multiple cases.

Responsibilities

• Perform intake with required admission documentation and referrals.
• Develop housing plans with short-term and long-term objectives.
• Provide mobile case management from a harm-reduction, strength-based model.
• Offer financial assistance per program policies.
• Develop discharge plans addressing housing, economic and emotional stability.
• Attend supervisory and team meetings as directed.
• Coordinate with community partners to ensure access and follow-through.
• Evaluate progress and adjust plans as needed.
• Maintain relationships with community agencies for comprehensive services.
• Collaborate with staff to meet grant outcomes.
• Provide case management across Moorhead, Bemidji, St. Cloud, and nearby areas.
