Job summary

Mental Health America of Los Angeles (MHALA) is seeking a Veteran Healthcare Navigator to deliver veteran-centered services focused on healthcare access, housing stability, and coordination of supportive services for Veterans and their families enrolled in the SSVF program.

Under supervision, the Navigator collaborates with VA, HUD-VASH, and community partners to reduce barriers to care and housing, while supporting case management and administrative duties as needed.

Qualifications

• Bachelor’s degree in human services, preferred
• Veterans encouraged to apply
• One year of providing psychiatric rehabilitation services to a disabled population, low income and/or unhoused population preferred.

Responsibilities

• Provide veteran-centered healthcare navigation and housing stability support.
• Coordinate referrals and warm hand-offs to VA Medical Centers and community providers.
• Maintain secure client records and HMIS documentation with 24-hour turnaround.
• Assist enrollment in VA healthcare and public benefits to promote continuity of care.
• Collaborate with HUD-VASH, SSVF and partners to reduce barriers to care and housing.
• Engage in team meetings and program operations to ensure seamless service delivery.
